Qualtrics – Qualtrics is a web-based survey tool available for free to all faculty, staff and students. To access Qualtrics, visit wvu.qualtrics.com and login using your WVU Login credentials to claim a Qualtrics account. If this is your first time using Qualtrics, click “I Do Not Have An Account” and an account will instantly be created and associated with your Login.
Google Forms via Google Drive – Google Forms is a free service available with your Google Drive account. The service includes a very simple and intuitive form builder, integration of submissions with Google Spreadsheets, reporting tools, and support for data export to several popular formats.

These are three examples among many available web form services. We encourage form publishers to research the available options and select the service that will best meet their project requirements and departmental budget.
